Most plastic surgeons choose to place the implants above the muscle (underneath the breast glands). This is partly due to a better cosmetic result, and also because of the reduced risk of discomfort and of bleeding after the surgery. Particularly if the breast is sagging a little, it would most often be preferable to place the implant above the muscle. This tightens the breast, and the unfortunate double contour is also avoided. Placing the implant above the muscle can also sometimes save the patient from having to have a breast lift (which usually leaves large, unsightly scars as opposed to a breast enlargement). If the patient is very slim, and if the ribs are prominent, it will be best to place the implants below the muscle. This is because a very slim woman does not have enough "upholstery" on the upper body, meaning that the edge of the implants would often become visible, if the implants were placed on top of the muscle. The decision on whether to place the implant above or below the muscle will be discussed with the plastic surgeon at the initial consultation.
